Comparison of coercive practices in worldwide mental healthcare: overcoming difficulties resulting from variations in monitoring strategies

Background
Coercive or restrictive practices such as compulsory admission, involuntary medication, seclusion and restraint impinge on individual autonomy. International consensus mandates reduction or elimination of restrictive practices in mental healthcare. To achieve this requires knowledge of the extent of these practices.
AimsWe determined rates of coercive practices and compared them across countries.
MethodWe identified nine country- or region-wide data-sets of rates and durations of restrictive practices in Australia, England, Germany, Ireland, Japan, New Zealand, The Netherlands, the USA and Wales. We compared the data-sets with each other and with mental healthcare indicators in World Health Organization and Organisation for Economic Cooperation and Development reports.
ResultsThe types and definitions of reported coercive practices varied considerably. Reported rates were highly variable, poorly reported and tracked using a diverse array of measures. However, we were able to combine duration measures to examine numbers of restrictive practices per year per 100 000 population for each country. The rates and durations of seclusion and restraint differed by factors of more than 100 between countries, with Japan showing a particularly high number of restraints.
ConclusionsWe recommend a common set of international measures, so that finer comparisons within and between countries can be made, and monitoring of trends to see whether alternatives to restraint are successful. These measurements should include information about the total numbers, durations and rates of coercive measures. We urge the World Health Organization to include these measures in their Mental Health Atlas.

Elie Halévy became famous as a historian of England in the years before World War I, due to his lectures on England at the Ecole libre des sciences politiques, his three-volume analysis of utilitarianism published between 1901 and 1904, a 1906 article on the birth of Methodism, and the 1912 book L’Angleterre en 1815. In these last two works he argued—in what became known as the Halévy thesis—that English Protestantism, and especially the evangelical forms of English Protestantism associated with Methodism, were a key element of Britain's sociopolitical stability. This deep-seated religiosity, he argued, was supportive of British liberalism and British philanthropy; it was responsible for an England that, in his own words, “governs itself, in place of being governed from above.”

The history of French liberalism is undergoing a renaissance. For much of the twentieth century, it was viewed with disdain, as insufficiently “engaged,” as too tentative in its demands for social reform, as overly optimistic concerning the progress of reason and science. Scholarship during the past three decades has challenged these views, though it is notable that there is still, to my knowledge, no general history of French liberalism that goes past the consolidation of the Third Republic in the late 1870s. Part of the ongoing reassessment has been the consequence of the decline of revolutionary illusions and of marxisant frameworks of analysis following 1968, reinforced by the more general decline of the left following the end of the Cold War in 1989 and the implosion of the Soviet Union in 1991. Another element contributing to this reassessment has been the emergence of more nuanced definitions of “liberalism,” ones that are not limited to legal (civil liberties), political (constitutionalism), and/or economic (free trade) dimensions. Equally important, scholars are insisting, are conceptions of science, of religion, of the role of the state, of solidarity, of sociability, of moeurs, of identity, of gender, of the self.

To be GOVERNED is to be kept in view, inspected, spied upon, directed, legislated,regulated, prosecuted, indoctrinated, preached at, controlled, estimated, valued,censured, commanded by those who do not have the right, the wisdom, northe virtue to do so . . . To be GOVERNED is to be at each operation, at eachtransaction, at each movement, noted, registered, counted, taxed, stamped, measured,numbered, assessed, licensed, authorised, annotated, admonished, forbidden,reformed, corrected, punished. It is, under pretext of public utility, and in thename of the general interest, to be placed under contribution, trained, ransomed,exploited, monopolised, extorted, pressured, mystified, robbed; then, at the slightestresistance, at the first word of complaint, to be reprimanded, fined, run down, harassed,tracked, abused, clubbed, disarmed, bound, imprisoned, shot, machine-gunned, judged,condemned, deported, sacrificed, sold, betrayed: and, to top it all off, mocked,ridiculed, outraged, dishonoured. Summary
Benjamin Constant’s relations with women have been a topic of endless fascination and attention. Almost every scholar of Constant’s novel Adolphe has given in to the temptation to identify the real-life woman who was the model for the novel’s main female protagonist Ellénore. (The principal candidates are Charlotte von Hardenberg, Germaine de Staël, and Anna Lindsay.) And many have insisted that the fictional Adolphe is a more or less transparent representation of Constant himself. Although such identifications are informative, they are less illuminating than analyzing how the important women in Constant’s life influenced his reflections on interpersonal relationships, human sentiments, and the importance of both for understanding society and politics.
Constant had casual and serious relationships with many women, and married twice. In 1789, as a young conseiller de Légation at the court of Brunswick, he married Wilhemine Luise Johanne “Minna” von Cramm, a lady-in-waiting. Due to Constant’s absences, Minna’s infidelities, and general incompatibility, by 1791 the marriage was in serious trouble; their separation in 1793 was legally codified in 1795. In 1808, he married another German woman, Charlotte von Hardenberg, with whom he remained until his death in 1830. All the evidence suggests that this was the most emotionally stable relationship of his life.

The cultural world of French elites was profoundly shaken by the long sequence of events we now refer to as the French Revolution. The legal, political, and social world in which elites moved was transformed by successive revolutionary regimes and, more generally, by what Benjamin Constant called “the torrent” of change that began with the Revolution in 1789. Individuals experienced excitement, hope, and enthusiasm, but also suffered trauma, insecurity, and loss. The effects of these experiences on elite sociability and culture are difficult to characterize precisely. Michelle Perrot and Anne Martin-Fugier have written that there was a tendency on the part of French elites during the nineteenth century to retreat increasingly into the sanctuary of their homes, where they practiced more exclusionary forms of sociability, and simultaneously adopted more defensive forms of politics. Alain Corbin reminds us that the vulnerability that accompanied the progress of individuation in nineteenth-century France frequently manifested itself as an obsession with self-scrutiny, as represented by the enriched forms of interior monologue found in diaries and in Romantic poetry and literature. This retreat and concomitant self-scrutiny can be seen as a further refinement of the delicacy and sense of shame—of what we call modesty—that Norbert Elias suggested over sixty years ago was a central characteristic of the advance of European manners.
